To get your payment amount due take the package price times your tax rate to get your total. Subtract what you have paid so far for your balance. Divide balance in half for 8 week payment and 3 week payment. Must be paid in full by three weeks prior to ceremony. Print form below, fill out and mail to address above. Payments over the phone will not be accepted. You can also use this form to purchase the High Rez DVD. Just make a note at the bottom what your payment is for.
