Album Approval / Changes Form
ALBUM CHANGES FORM Final Changes Must be received by November 1st for Christmas Delivery - No matter when
you submitted order- final approval for print must have been received by November 1st for possible Christmas Delivery.
No exceptions! We have no control over this, we have to go by lab guidelines. Orders for albums that we receive the
approval form after Nov. 1st are usually done in January, depending upon the work load of the lab during the holiday
season.
Once your album has been posted online you MUST fill out the form below to approve and or to make changes to your album. This form can only be mailed in. Changes or approval cannot be made by phone. This is to assure that your album gets engraved properly, shipped to the correct address, and any changes made, are made to your specifications.
